Il a r\u00e9cemment \u00e9t\u00e9 d\u00e9montr\u00e9 que l\u2019administration de Lactobacillus rhamnosus durant la petite enfance peut contribuer \u00e0&nbsp;<strong><strong>r\u00e9duire l\u2019incidence des caries dentaires<\/strong><\/strong>.&nbsp;<\/p>\n\n\n\n<div style=\"height:48px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Composition et caract\u00e9ristiques techniques<\/h2>\n\n\n\n<div style=\"height:32px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><strong>Lactobacillus rhamnosus SP&nbsp;1<\/strong>&nbsp;est une souche probiotique propri\u00e9t\u00e9 de&nbsp;<strong>Sacco S.r.l.<\/strong>, d\u00e9pos\u00e9e sous le num\u00e9ro&nbsp;<strong>DSM 21690<\/strong>, et identifi\u00e9e par s\u00e9quen\u00e7age de l\u2019ADN codant pour l\u2019ARNr 16S par PCR&nbsp;; le typage de la souche a \u00e9t\u00e9 effectu\u00e9 par \u00e9lectrophor\u00e8se en champ puls\u00e9 (PFGE). La souche bact\u00e9rienne non-OGM est \u00e0 Gram positif, microa\u00e9rophile et h\u00e9t\u00e9rofermentaire facultative. Elle est \u00e9galement&nbsp;<strong>sans gluten<\/strong>,&nbsp;<strong>sans soja<\/strong>&nbsp;et sans&nbsp;<strong>d\u00e9riv\u00e9s de lait<\/strong>. Conform\u00e9ment aux instructions de l\u2019EFSA, des tests ont \u00e9t\u00e9 r\u00e9alis\u00e9s afin d\u2019exclure la pr\u00e9sence de traits de r\u00e9sistance aux antibiotiques acquis et \u00e9galement potentiellement transmissibles. L\u2019<strong>\u00e9tude<\/strong>&nbsp;(5) a port\u00e9 sur 205&nbsp;enfants fr\u00e9quentant 16&nbsp;\u00e9coles maternelles inscrites au Programme national de nutrition de Santiago du Chili, qui \u00e9taient les plus expos\u00e9s aux caries dentaires&nbsp;; 123&nbsp;enfants ont bu du lait contenant Lactobacillus rhamnosus SP&nbsp;1, les 82&nbsp;autres ont bu du lait contenant un placebo.<\/p>\n\n\n\n<p>Pour participer \u00e0 l\u2019\u00e9tude, les&nbsp;<strong>enfants devaient \u00eatre en bonne sant\u00e9<\/strong>&nbsp;et ne pas pr\u00e9senter d\u2019intol\u00e9rance au lait ou d\u2019allergies alimentaires. Les \u00e9coles qui constituaient le groupe ayant test\u00e9&nbsp;<strong>Lactobacillus rhamnosus SP&nbsp;1<\/strong>&nbsp;comme compl\u00e9ment et celles qui constituaient le groupe t\u00e9moin ont \u00e9t\u00e9 choisies au hasard.&nbsp;Ni le personnel des \u00e9coles concern\u00e9es, ni les familles des enfants ou les m\u00e9decins n\u2019ont \u00e9t\u00e9 inform\u00e9s des \u00e9coles maternelles qui recevraient du lait enrichi en Lactobacillus rhamnosus SP&nbsp;1 et de celles qui recevraient du lait t\u00e9moin sans probiotique. Chaque jour de classe, les enfants ont re\u00e7u du lait compos\u00e9 de lait en poudre partiellement \u00e9cr\u00e9m\u00e9 dissous et donc adapt\u00e9 \u00e0 l\u2019ajout de Lactobacillus rhamnosus SP&nbsp;1 ; en parall\u00e8le, du lait placebo, consomm\u00e9 de la m\u00eame mani\u00e8re, a \u00e9galement \u00e9t\u00e9 donn\u00e9 au go\u00fbter l\u2019apr\u00e8s-midi (150&nbsp;ml de lait). Le lait a \u00e9t\u00e9 enrichi de&nbsp;<strong>1,5&nbsp;milliard de ferments lactiques vivants \u00e0 l\u2019aide d\u2019une culture en poudre lyophilis\u00e9e de Lactobacillus rhamnosus SP&nbsp;1<\/strong>. Le test a dur\u00e9 10&nbsp;mois et chaque jour scolaire, le personnel des 16&nbsp;\u00e9coles a rempli un journal consignant la pr\u00e9sence des enfants afin de garantir le respect du protocole d\u2019\u00e9tude.<\/p>\n\n\n\n<p>L\u2019<strong>exp\u00e9rience<\/strong>&nbsp;\u00e9tait randomis\u00e9e, en triple aveugle, contr\u00f4l\u00e9e par placebo et \u00e0 groupes parall\u00e8les. Les&nbsp;<strong>crit\u00e8res internationaux ICDAS<\/strong>&nbsp;pour la d\u00e9tection visuelle et tactile des l\u00e9sions carieuses et pour la description de la gravit\u00e9 des l\u00e9sions ont \u00e9t\u00e9 utilis\u00e9s. Les enfants ont \u00e9t\u00e9 examin\u00e9s au d\u00e9but et \u00e0 la fin de l\u2019\u00e9tude par deux dentistes form\u00e9s par des examinateurs internes et externes afin de garantir des r\u00e9sultats d\u2019examen coh\u00e9rents et comparables.<\/p>\n\n\n\n<p>Les observations ont montr\u00e9 une pr\u00e9sence de caries l\u00e9g\u00e8rement inf\u00e9rieure, mais toujours significative, dans le groupe ayant re\u00e7u Lb. rhamnosus SP&nbsp;1 (54,4&nbsp;%) par rapport au groupe t\u00e9moin (65,8&nbsp;%). Il a \u00e9galement \u00e9t\u00e9 r\u00e9v\u00e9l\u00e9 qu\u2019apr\u00e8s deux mois sans nouvelle administration de Lb. rhamnosus SP&nbsp;1 (soit un total de 12&nbsp;mois apr\u00e8s le d\u00e9but du test), les&nbsp;<strong>l\u00e9sions carieuses chez les enfants ayant bu du lait additionn\u00e9 de Lb. rhamnosus SP&nbsp;1 \u00e9taient significativement plus faibles<\/strong>&nbsp;(9,7&nbsp;%) que dans le groupe t\u00e9moin (24,3&nbsp;%), ce qui d\u00e9montre que la probabilit\u00e9 de manifester des l\u00e9sions dentaires r\u00e9sultant de caries pendant la p\u00e9riode suivant l\u2019administration de la bact\u00e9rie \u00e9tait significativement r\u00e9duite.<\/p>\n\n\n\n<p>Le mode d\u2019action pourrait \u00eatre li\u00e9 au fait que&nbsp;<strong>Lb. rhamnosus SP&nbsp;1 interagit avec le biofilm buccal en modifiant sa composition au profit de bact\u00e9ries qui produisent moins d\u2019acide organique<\/strong>, permettant un meilleur \u00e9quilibre entre le pH de la salive et celui de la plaque dentaire. Cette \u00e9tude a par ailleurs d\u00e9montr\u00e9 une influence positive m\u00eame apr\u00e8s deux mois sans autre apport de bact\u00e9ries probiotiques&nbsp;; d\u2019autres \u00e9tudes avec des lactobacilles probiotiques ont d\u00e9montr\u00e9 qu\u2019ils n\u2019\u00e9taient que transitoirement pr\u00e9sents dans la salive, et donc qu\u2019un&nbsp;<strong>apport quotidien peut \u00eatre recommand\u00e9 pour limiter le d\u00e9veloppement de caries dentaires<\/strong>&nbsp;chez les&nbsp;<strong>enfants<\/strong>&nbsp;d\u2019\u00e2ge pr\u00e9scolaire.&nbsp;<br>&nbsp;<\/p>\n\n\n\n<div style=\"height:48px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">S\u00e9curit\u00e9<\/h2>\n\n\n\n<div style=\"height:32px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><strong>Lactobacillus rhamnosus SP&nbsp;1<\/strong>&nbsp;est un micro-organisme&nbsp;<strong>s\u00fbr<\/strong>&nbsp;reconnu&nbsp;<strong>QPS<\/strong>&nbsp;(pr\u00e9somption d\u2019innocuit\u00e9 reconnue, Qualified Presumption of Safety) par l\u2019EFSA.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">&nbsp;<\/h2>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Applications et modes d\u2019utilisation<\/strong><\/h2>\n\n\n\n<div style=\"height:32px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><strong>Lactobacillus rhamnosus SP&nbsp;1<\/strong>&nbsp;peut \u00eatre utilis\u00e9 comme&nbsp;<strong>compl\u00e9ment probiotique<\/strong>, ou ajout\u00e9 \u00e0 des produits alimentaires probiotiques.
